Quick Guide to Phase II Instructions

-Text (562) 249-1527 (La Habra) or (562) 372-6750 (Chino Hills) upon arrival for instructions.

-Must wear a mask and maintain Social Distancing during your visit.

-You will be asked to wash your hands for 20 seconds and use a mouthrinse upon arrival.

-Parents will be asked to wait outside or in their car until your child is ready for pick-up to limit the number of groups in the office.

-Special hours will be reserved for patients that need a caregiver during the appointment.
