New Patients
Your first visit to Center for Pediatric Dentistry involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There's also background information about our committed staff and our first visit procedures. You can also save some time by printing out and completing the patient forms in advance of your appointment.

What To Expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible recommendations for your child. It also will help relieve any unnecessary anxiety you may be feeling. We will send you a reminder text a few days prior to your appointment. Please respond to the text to confirm and reserve your appointment. We will also send a link to pre-fill our New Patient Information Form and Medical & Dental History form. You may also download from the website and bring it in. We will ask to scan your ID and dental insurance card upon your visit. Any deductibles or copayments are due at the time of the visit. Take some time to review our staff page and familiarize yourself with the doctors. We look forward to your first visit.

If you are being referred from another office, please bring your Referral Form and a copy of any X-rays to your consulation appointment. Your office may also email those documents securely if your choose. Please contact us for our direct email address.
